Are compact flash cards hot swappable under all circumstances?

In a PCMCIA adapter, must the PC be off to pull the adapter?

In USB reader connected to desktop, can the flash cards be removed without unplugging the USB reader?

I don't know the rules, so can't play the game!

I searched for a treatise on flashing, but came up empty. Any flashers out there that can point me to a guide?

compact flash cards should not be hotswapped while reading or writing, or when a program has a hold on them (even when its not actively doing something)IE> I have to tell Jumpshot to turn my Lexar card reader loose,
and a ICON is placed by the program for that purpose on my taskbar
