To add an item to right click
how to add an item to right click (i.e right click on desktop-->New-->Folder,text document etc)

Create a shortcut to whatever you want to "open" and drag it into your "Send to" folder.

It may thus need a registry edit, but I think you can also do it through the file associations because I'm sure I've done this to allow new documents to be selected in both Word97 and Word2000 format, when both have been running on the same pc.

Which application do you want listed there?

Do you ever actually read the responses to the threads you post? You keep posting the same questions in new threads and don't respond to the comments that people leave. If you simply want to bring your question to the top again, you can reply to your original post and it will rise to the top. Posting the same question in multiple threads is considered rude around here, as mjc already pointed out to you...

Yes..I saw this and thought, "I already searched for this, and I am not searching again." It was a good link with clear instructions, and I have added it to Favorites for future reference.
