    computer repair forms

    Does anyone know of a site that has examples of computer or electonic repair forms (for Free) that someone thinking of starting a business might take a look at.

    I think he means an invoice. Something a tech would fill out while doing a job.

    If I remember right (I'm at my day job now - repairs is a very sporadic weekend job for me outside of family and friends), ours has blanks for customer info, system info, description of problem, action taken, start time, end time, total hours, total labor charge, total parts charge, total charge, method of payment, and a "not responsible for loss of data" waiver (the waiver has its own signature line - we rarely make the customer sign that unless we're taking it with us or their data's pretty much gone already, like if the hard drive is dead).

    And, of course, company logo and contact info.
